Family Scholarships to CWD's Friends for Life Conference

The Education for Children with Diabetes Foundation will provide financial assistance for families from the United States or Canada to attend the Children with Diabetes Friends for Life Conference at the Renaissance Orlando Resort at Sea World, from July 23-27, 2008. The conference is a gathering of world-renowned clinicians, researchers, physicians, adults, children and families with diabetes. It offers many opportunities to learn the most current information in diabetes care.

Family scholarships cover the cost of conference registration, hotel stay, Universal Studios tickets for Saturday as part of the conference group, and limited meals at the conference. Families must provide their own transportation to the conference.

What is Covered?

Each scholarship will cover the following expenses (for a family of up to 4 people):

  • Conference registration, which includes educational sessions, activities, and unlimited access to vendor exhibits
  • Meals during the conference, including Opening Reception (Wednesday PM); breakfast, lunch and snacks on Thursday & Friday; Farewell Breakfast (Sunday AM)
  • One guest room (for up to four guests) at Renaissance Orlando Resort from July 23 through July 27, 2008 (four nights)
  • 2008 CWD conference t-shirt
  • Special Events
  • First Timers Reception (Wednesday Afternoon)
  • Family and Friends Banquet (Thursday PM/requires separate entrance ticket)
  • Dance and Social Evening (Thursday PM following the Banquet)
  • Teen Day at a theme park offsite (Friday Teens 13-17)
  • Teen Dinner Dance (Friday PM Teens 13-17)
  • Tween Night (Friday PM Tweens 10-12)
  • Dessert with CWD Faculty (Friday PM)
  • Education for Children with Diabetes Foundation (ECWDF) BBQ Lunch at Universal Studios City Walk (Saturday AM/requires separate entrance ticket)
  • Universal Studios Park Entrance (Saturday only, as part of the Benefit above)

What is Not Covered?

The following items will not be covered under the scholarship:

  • Transportation to and from the conference
  • Meals not outlined above
  • Theme park tickets other than Saturday at the Benefit BBQ at Universal Studios
  • Incidental charges (i.e., phone, room service, etc) incurred during the stay (each family will be required to provide the hotel a credit card for any incidental charges)
  • Expenses for extended family members and/or friends

Who is Eligible?

The Foundation seeks to award conference scholarships to families from the United States or Canada with at least one child who has Type 1 diabetes, age infant to 18.

Eligible families must complete a typed application (not handwritten), postmarked by April 15, 2008. Applications will be evaluated based on the individual family's:

  • Interest in better caring for their child with diabetes
  • Enthusiasm for sharing their diabetes knowledge with others in their community
  • Track record of involvement in diabetes education outreach
  • Financial need

Each family must have at least one child (age infant to 18 years old) who has type 1 diabetes.

Applicants from the United States are required to provide a copy of their 2007 Federal Tax Form 1040, copy of W-2, as well as their last two pay stubs (for each parent if both parents work). Applications must be submitted with all supporting material at the same time. Incomplete applications will not be processed.

Applicants from Canada are required to provide a copy of their 2007 Notice of Assessment for all adults in the household. Applications must be submitted with all supporting material at the same time. Incomplete applications will not be processed.

Recipients are expected to attend a full complement of educational sessions and other events held during the CWD conference. This includes a total of 8 sessions (four on Thursday and four on Friday). Each parent attending will also be required to perform 2-3 hours of volunteer activities during the conference, assigned by CWD staff. Park tickets will not be distributed until this requirement is fulfilled and validated. Additionally, one of the parents will be asked to complete a brief written evaluation of the conference and the scholarship program at the conclusion of the conference. It is encouraged that both parents of any two-parent family receiving a scholarship attend.

Initial scholarships will be awarded no later than May 31, 2008. Applicants will be notified via email by May 31, 2008 as to whether their application was accepted. Applicants must furnish email addresses in order to be notified. Sponsorship funds will be disbursed directly to Children with Diabetes to cover conference costs. Children with Diabetes will make hotel reservations for the sponsored families.

The Foundation reserves the right to award additional scholarships, after the deadline and notification dates, if funds become available.
